Immediate Next Steps:
Required Onboarding Documents
Please complete this form which gathers important contact information to complete the necessary onboarding documents.
The following documents will be pre-filled by TadHealth then routed to your organization’s designated signer. Please review the description of each document to determine who the best individual would be to sign the forms.
Provider Participation Agreement (PPA): Your required agreement with the Department of Health Care Services (DHCS) to participate in CYBHI and bill for covered services. Your LEA’s Medi-cal Fee for Service enrollment occurs. once the PPA is completed. The PPA outlines provider responsibilities and authorizes payment for eligible services once approved.
Note: In order to proceed with onboarding, this document must be signed first. Please review and sign at your earliest convenience.
DHCS Tri-Party Data Use Agreement (DUA): The first of two DUAs to be signed by your LEA, TadHealth, and DHCS. This DUA enables us to send basic student identifiers to DHCS in exchange for Medi-Cal eligibility and coverage data, used to confirm billing eligibility and outlines each party’s responsibilities for privacy and compliance.
Carelon Tri-Party Data Use Agreement (DUA): The second required DUA signed by Carelon in addition to your organization and TadHealth. Establishes the legal framework for your organization, providers, and TadHealth to securely share student services and claims data for CYBHI and outlines each party’s responsibilities for privacy and compliance.
PR Provider Form: A communication opt-in that authorizes Carelon to contact your organization and its providers with CYBHI news and updates. We pre-fill this form and send it to you for review. Once approved, your organization emails it directly to Carelon at [email protected].
Provider Connect: A required Carelon step that sets up a backup secure file transfer (SFTP) connection between your organization and Carelon. TadHealth maintains its own SFTP connection and sends files on your behalf, so this is simply a backup — signed after the PPA and both DUAs are complete.
